• Key Responsibilities:
• Engage members through telephone outreach and home visits, discussing care plans, addressing questions, and providing ongoing education.
• Coordinate care activities, collaborating with healthcare providers, community partners, and members to accommodate changes and progress.
• Serve as a support for member and provider inquiries, ensuring continuity of care and facilitating services.
• Document member records, maintaining compliance with state and regulatory requirements.
• Identify needs, make referrals, and provide education on benefits and resources available.
• Perform other duties as assigned and comply with all policies and standards.

Qualifications:

• High School diploma or GED required.
• 1 – 2 years of related experience.
• For Arkansas Total Care plan, a Bachelor's degree in social science/health-related field or relevant experience coordinating care for specific populations. Safety-sensitive position requiring a driver's license, background checks, and drug screen.
• Preferred: Experience working with behavioral health and developmental disabilities populations. Residency in or willingness to travel within specific Arkansas counties.

Compensation and Benefits:

• Pay range: $17.84 - $28.02 per hour.
