Join Our Dynamic Home First Team in Tweeddale

The Home First Service, a collaborative initiative with acute and community teams, seeks an experienced Healthcare Support Worker (HCSW) to join our Tweeddale locality team on a fixed-term basis from 01/10/2026 to 30/04/2027. Our focus is on prevention, anticipation, and supported self-management, working alongside Hospital at Home, District Nursing, and other health and social care services.

• Provide high-quality, person-centred care in patients' homes
• Work collaboratively with multidisciplinary teams, patients, and families
• Support service delivery across Central, Teviot & Tweeddale localities as needed
• Flexible hours: 08:00 - 20:30 over 7 days, including weekends
• Full UK driving licence with business insurance required
