Geriatric Telemedicine Consultant

Provide comprehensive, patient-centered primary care through telemedicine, focusing on the unique needs of older adults. Evaluate, diagnose, and manage a wide range of acute and chronic medical conditions, including multimorbidity, frailty, dementia, and polypharmacy. Deliver evidence-based care, optimize healthcare resources, and promote healthy aging.

Patient Consultations and Management:

• Deliver virtual assessments, diagnoses, treatments, and follow-ups for family medicine, adults, and pediatrics, with a focus on older adults.
• Conduct comprehensive clinical evaluations, including history taking, virtual physical assessments, medication reviews, and geriatric assessments.
• Develop individualized, evidence-based management plans, including medication optimization, preventive care, and referrals.
• Manage complex geriatric conditions, ensuring continuity and coordination of care.
• Collaborate with multidisciplinary teams, caregivers, and community services.
• Utilize digital health technologies for safe, high-quality virtual consultations and accurate documentation.
• Promote preventive care, chronic disease management, and value-based care principles.

Quality Assurance and Compliance:

• Communicate effectively with patients, families, and caregivers.
• Ensure compliance with clinical, ethical, regulatory, and telemedicine standards.
• Maintain accurate, timely, and complete medical documentation.

Telehealth Platform Proficiency:

• Maintain accurate, complete, and timely clinical documentation in the EHR.
