NHS Borders is seeking a dedicated and compassionate CTAC Staff Nurse to join our dynamic Community Treatment and Care (CTAC) team, serving the communities of Peebles, Innerleithen, and West Linton. Key Responsibilities
As a CTAC Staff Nurse, you will play a vital role in providing exceptional care to patients in their own homes or community settings. Your responsibilities will include:
- Delivering Person-Centred Care: Assessing, planning, implementing, and evaluating patient care plans to ensure the highest standards of treatment and support.
- Autonomous Practice: Working independently while collaborating closely with multidisciplinary teams to achieve optimal patient outcomes.
- Effective Communication: Building strong relationships with patients, their families, and healthcare professionals through clear and empathetic communication.
- IT System Proficiency: Utilising electronic health records and other digital tools to maintain accurate and up-to-date patient information.
- Travel and Mobility: Providing care across a wide geographical area, requiring access to a vehicle and a full driving licence.
Ideal Candidate Profile
