Key Responsibilities

As a CTAC Staff Nurse, you will play a vital role in providing exceptional care to patients in their own homes or community settings. Your responsibilities will include:

• Delivering Person-Centred Care: Assessing, planning, implementing, and evaluating patient care plans to ensure the highest standards of treatment and support.
• Autonomous Practice: Working independently while collaborating closely with multidisciplinary teams to achieve optimal patient outcomes.
• Effective Communication: Building strong relationships with patients, their families, and healthcare professionals through clear and empathetic communication.
• IT System Proficiency: Utilising electronic health records and other digital tools to maintain accurate and up-to-date patient information.
• Travel and Mobility: Providing care across a wide geographical area, requiring access to a vehicle and a full driving licence.

Ideal Candidate Profile

• A valid nursing registration with the Nursing and Midwifery Council (NMC).

• The ability to work autonomously, manage time efficiently, and adapt to changing priorities.
• Proficiency in using IT systems and electronic health records.
• A full driving licence and access to a vehicle, as the role involves regular travel across the community.
