  • Provide ongoing care for patients with multi-system diseases through structured telemedicine pathways, including monitoring treatment adherence and outcomes via remote patient monitoring systems.
  • Coordinate follow-up visits, both virtual and in-person, as clinically indicated, and develop personalized treatment plans encompassing medication management, specialist referrals, and lifestyle modifications.
  • Document all findings and treatment plans meticulously in patient records to ensure continuity of care.
• Collaboration and Communication:
  • Collaborate with consultants, subspecialists, and allied health professionals through digital platforms to facilitate seamless care transitions.
  • Deliver health education and counseling virtually, empowering patients in self-management and preventive care strategies.

Qualifications and Experience

To excel in this role, candidates must possess the following qualifications and attributes:

• A medical degree (MD or equivalent) with a valid license to practice medicine.
• Completion of, or current enrollment in, a recognized internal medicine residency program, with a strong foundation in adult medicine.
