Position Overview

The Registrar – Internal Medicine (Home Healthcare) plays a pivotal role in delivering advanced medical care to adult patients within the comfort of their homes. This position is designed for a highly skilled medical professional who specializes in the assessment, diagnosis, and management of a diverse range of acute and chronic medical conditions. The Registrar is responsible for developing and implementing individualized treatment plans, coordinating multidisciplinary care teams, and ensuring adherence to the highest standards of clinical practice. Key Responsibilities

Patient Consultation and Clinical Case Management

• Conduct thorough medical assessments during home visits and virtual consultations to evaluate patient health status and identify medical needs.
• Diagnose and manage a wide spectrum of acute and chronic internal medicine conditions, ensuring timely and accurate clinical decision-making.
• Develop and implement individualized treatment plans that include medication management, diagnostic investigations, specialist referrals, and structured follow-up schedules.
• Monitor patient progress through regular evaluations and adjust treatment plans based on clinical findings to optimize health outcomes.
• Identify patients requiring escalation of care or hospital admission and facilitate prompt referrals to appropriate healthcare facilities.
• Provide medical consultations via telemedicine platforms to enhance accessibility and convenience for patients.
• Ensure meticulous, complete, and timely documentation of all medical assessments, diagnoses, treatment plans, and follow-up notes in accordance with evidence-based practice and organizational standards.
• Educate patients and their caregivers on disease management, medication adherence, preventive care strategies, and lifestyle modifications to promote long-term health and well-being.
• Collaborate closely with consultants, specialists, nurses, and allied healthcare professionals to ensure coordinated and holistic patient care.

Patient Safety and Satisfaction
