Position Overview

Johns Hopkins Aramco Healthcare (JHAH) is seeking a highly motivated and experienced Case Manager 2 to join our multidisciplinary team in Dhahran. Key Responsibilities

The Case Manager 2 will be responsible for a wide range of duties designed to enhance patient care and streamline healthcare processes. Your core responsibilities will include:

• Care Coordination: Develop, implement, and monitor individualized care plans for patients, ensuring alignment with clinical guidelines and best practices. Collaborate with physicians, nurses, and other healthcare professionals to deliver integrated care.
• Patient Advocacy: Serve as a strong advocate for patients, addressing their concerns, facilitating communication between patients and healthcare providers, and ensuring their rights and needs are met throughout their healthcare journey.
• Insurance Liaison: Work closely with insurance companies to verify coverage, obtain prior authorizations, and facilitate claims processing. Act as a bridge between patients and payers to resolve any discrepancies or barriers to care.
• Resource Management: Identify and connect patients with appropriate community resources, support services, and financial assistance programs to address social determinants of health and improve overall well-being.
• Documentation and Reporting: Maintain accurate and detailed records of patient interactions, care plans, and progress. Generate reports as required to track outcomes, identify trends, and support quality improvement initiatives.
• Education and Support: Provide patients and their families with education on chronic disease management, preventive care, and healthy lifestyle choices. Empower them to take an active role in their health and wellness.
• Compliance and Ethics: Ensure adherence to all regulatory standards, including HIPAA, and uphold the highest ethical standards in all interactions. Stay updated on industry regulations and best practices to maintain compliance.

Qualifications and Skills

To excel in this role, you must possess the following qualifications and competencies:

• Education: Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field. A Master’s degree is preferred.
• Experience: Minimum of 3-5 years of experience in case management, healthcare coordination, or a related field. Prior experience in a hospital or clinical setting is highly desirable.
• Certifications: Active certification in case management (e.g., CCM) is a plus. BLS/CPR certification may be required.
• Technical Skills: Proficiency in electronic health records (EHR) systems, Microsoft Office Suite, and case management software. Strong analytical and problem-solving skills.
• Soft Skills: Excellent communication, interpersonal, and organizational abilities. Demonstrated leadership, critical thinking, and the capacity to work effectively in a fast-paced, collaborative environment.
• Language Proficiency: Fluency in English is required. Proficiency in Arabic is advantageous.
