Key Responsibilities:

• Medication Safety & Dispensing: Accurately interpret and verify prescriptions, ensuring precise dosage calculations, proper labeling, and adherence to clinical guidelines. Dispense medications with meticulous attention to detail, minimizing errors and mitigating risks to patient safety.
• Patient Counseling & Education: Deliver clear, compassionate, and evidence-based counseling to patients and caregivers on medication usage, potential side effects, and lifestyle adjustments. Empower patients with the knowledge to adhere to treatment plans and optimize therapeutic outcomes.
• Clinical Collaboration & Pharmacotherapy Optimization: Partner closely with physicians, nurses, and other healthcare providers to refine pharmacotherapy plans, monitor treatment efficacy, and address adverse drug reactions. Contribute to medication reconciliation processes to ensure continuity of care across transitions.
• Inventory & Compliance Management: Maintain accurate medication inventories, track expiration dates, and ensure optimal storage conditions to preserve drug efficacy. Stay current with local healthcare regulations, insurance protocols, and internal policies to guarantee full compliance.
• Quality Improvement & Documentation: Identify opportunities for process enhancements, document clinical interventions in electronic systems, and participate in quality assurance initiatives. Support data-driven decision-making to elevate the standard of care.
• Health Promotion & Community Engagement: Lead or participate in patient education programs, health screenings, and community outreach initiatives to foster preventive care and wellness.

• Technical Proficiency: Fluency in pharmacy information systems, electronic medical records (EMR), and office software. Adaptability to integrate new technologies and workflows.
• Communication & Collaboration: Exceptional interpersonal skills to engage with diverse patient populations, families, and multidisciplinary teams. Ability to translate clinical complexities into accessible, actionable information.
• Operational Excellence: Keen attention to detail, strong organizational skills, and the ability to manage high-pressure environments efficiently. Commitment to maintaining accurate records, inventory, and compliance documentation.
