Key Responsibilities:
- Individualised Care Planning: Develop and implement comprehensive, patient-centred treatment and recovery plans tailored to each individual’s unique needs, ensuring holistic support for their return home.
- Collaborative Practice: Work closely with the multi-disciplinary team (MDT) and third-sector agencies to advocate for patients, fostering a collaborative approach that prioritises recovery and well-being.
- Advocacy and Support: Champion the rights and dignity of patients, ensuring their voices are heard and their care aligns with their personal goals and aspirations.
- Excellence in Care: Deliver care that adheres to the highest clinical standards, demonstrating flexibility, resourcefulness, and strong communication skills both verbally and in writing.

About the Role:
The role is based at East Brig, located at Tweed Road, Galashiels, and operates on a permanent, part-time basis (30 hours per week). This position is suitable for both experienced nurses and those who are newly qualified, offering a supportive entry point into the field or an opportunity to refine and expand your expertise.
