Geriatric Care Specialist

We are seeking a dedicated Consultant – Family Medicine (Geriatrics) to provide comprehensive, patient-centered care to our older adult population. This role involves evaluating, diagnosing, and managing a wide range of acute and chronic medical conditions, with a focus on the unique needs of the geriatric population. The ideal candidate will deliver evidence-based care across both in-person clinic and telemedicine settings, ensuring continuity of care, medication optimization, and preventive care.

Responsibilities:

• Deliver comprehensive care through in-person and virtual consultations, assessing, diagnosing, treating, and following up on acute and chronic medical conditions.
• Conduct thorough clinical evaluations, including history taking, physical or virtual assessment, medication review, and geriatric assessment where appropriate.
• Determine when a patient's condition requires an in-person visit or can be safely managed virtually, and facilitate smooth transitions between the two.
• Develop and implement individualized, evidence-based management plans, including medication optimization, preventive care, lifestyle interventions, and referrals when indicated.
• Manage complex geriatric conditions, ensuring continuity and coordination of care across both channels.
• Collaborate with multidisciplinary healthcare teams, caregivers, and community services to deliver integrated, patient-centered care in-person and virtually.
• Utilize digital health technologies alongside standard in-clinic systems to provide safe, high-quality consultations while maintaining accurate and timely clinical documentation.
• Promote healthy aging through preventive care, chronic disease management, population health initiatives, and value-based care principles.
• Ensure compliance with clinical, ethical, regulatory, and telemedicine standards while maintaining accurate, timely, and complete medical documentation.
• Maintain current knowledge of evidence-based practice, in-clinic and telemedicine regulations, and geriatric care through continuous professional development.
• Contribute to clinical governance by participating in quality improvement initiatives, clinical audits, research activities, and regulatory compliance programs.
• Maintain proficiency in using the electronic health record (EHR) system in compliance with organizational policies and applicable regulations.
