Consultant - Family Medicine

Position Overview

We are seeking a highly skilled and dedicated Consultant in Family Medicine with a specialized focus on diabetology to join our dynamic telemedicine team. In this critical role, you will deliver comprehensive, patient-centered medical care to adult and pediatric patients, with a primary emphasis on the diagnosis, treatment, and long-term management of diabetes and related metabolic disorders. Additionally, you will address a wide spectrum of acute and chronic medical conditions, ensuring evidence-based care through collaborative, multidisciplinary approaches. The ideal candidate will be committed to optimizing clinical outcomes, enhancing patient experiences, and advancing value-based care principles within a telehealth framework.

Core Responsibilities

The Consultant - Family Medicine will be responsible for the following key functions:

• Patient Consultations and Management:
  • Provide comprehensive telemedicine care through virtual assessments, diagnoses, treatments, and follow-ups for diabetes, metabolic disorders, and other acute and chronic conditions.
  • Conduct thorough clinical evaluations, including detailed medical histories, virtual physical assessments, medication reviews, diabetes risk assessments, and interpretation of laboratory and diagnostic findings.
  • Develop and implement individualized, evidence-based management plans that encompass glycemic optimization, medication management, lifestyle modifications, preventive care, and specialist referrals as needed.
  • Manage patients with type 1 diabetes, type 2 diabetes, prediabetes, obesity, and other metabolic conditions, while proactively preventing, identifying, and addressing diabetes-related complications and comorbidities.
  • Collaborate with multidisciplinary healthcare teams—including endocrinologists, dietitians, diabetes educators, pharmacists, and primary care providers—to ensure seamless, coordinated, and patient-centered care delivery.
  • Leverage digital health technologies, such as remote patient monitoring and telemedicine platforms, to deliver safe, high-quality virtual care while maintaining precise and timely clinical documentation.
  • Champion preventive care, chronic disease management, population health, and value-based care initiatives to enhance glycemic control, reduce diabetes-related complications, improve patient outcomes, and optimize healthcare resource utilization.
• Quality Assurance and Compliance:
  • Communicate effectively with patients and their families, ensuring clear and concise explanations of diagnoses, treatment options, and follow-up care plans.
  • Adhere to all relevant medical regulations, ethical guidelines, and quality assurance standards specific to telehealth practice.
  • Maintain current knowledge of telehealth regulations and best practices, actively participating in quality improvement initiatives, research activities, and clinical audits to elevate patient care standards.
  • Regularly review and update compliance protocols, participate in mandatory training sessions, and collaborate with compliance officers to ensure adherence to regulatory standards.
  • Prepare accurate and comprehensive reports for audits and inspections, staying vigilant to changes in healthcare regulations and implementing necessary adjustments promptly.
• Professional Development:
  • Commit to continuous professional growth in family medicine, with a specialized focus on diabetology and best practices in both in-person and telehealth settings.
  • Attend industry conferences, workshops, and online courses to stay abreast of the latest advancements in diabetology, telehealth technologies, and evidence-based medical practices.
• Telehealth Platform Proficiency:
  • Maintain meticulous and compliant patient records in accordance with HIPAA regulations.
  • Demonstrate proficiency in utilizing designated telehealth platforms, including video conferencing, secure messaging, and electronic health record (EHR) integration.
  • Engage actively in training programs to enhance platform utilization and stay informed about updates, enhancements, and best practices. Collaborate with IT support to resolve technical issues efficiently.
• Patient Safety and Satisfaction:
  • Uphold the highest standards of patient safety across all telemedicine interactions and ensure a positive patient experience through compassionate, accessible, and high-quality care.

Qualifications and Skills

Candidates for this role should possess the following qualifications and competencies:

• Board certification or eligibility in Family Medicine, with a strong preference for additional certification or experience in Endocrinology or Diabetology.
• Minimum of three to five years of clinical experience in family medicine, with a demonstrated focus on diabetes management and metabolic disorders.
• Proven expertise in telemedicine, including familiarity with remote patient monitoring tools and digital health platforms.
• Exceptional communication skills, with the ability to convey complex medical information clearly and empathetically to patients and multidisciplinary teams.
