As a Clinical Pharmacist at Diaverum, you will collaborate closely with multidisciplinary healthcare teams, including nephrologists, nurses, and dietitians, to ensure the safe, effective, and evidence-based use of medications for patients undergoing dialysis and other renal therapies. Your expertise will be instrumental in improving medication adherence, preventing adverse drug reactions, and contributing to the overall quality of care delivered in our state-of-the-art dialysis centers.

In this role, you will:

• Develop and implement comprehensive medication management plans tailored to individual patient needs, ensuring optimal therapeutic outcomes and adherence to clinical guidelines.
• Conduct regular medication reviews, identify potential drug interactions, and recommend adjustments to enhance patient safety and efficacy.
• Educate and counsel patients, caregivers, and healthcare providers on medication therapy, fostering informed decision-making and improving health literacy.
• Participate in quality improvement initiatives, clinical research, and evidence-based practice projects to advance renal care standards.
• Collaborate with nephrology teams to monitor and evaluate patient responses to treatment, adjusting regimens as needed to achieve desired clinical outcomes.
• Contribute to the development of clinical protocols, policies, and guidelines that align with international best practices in nephrology and pharmacotherapy.
