Key Responsibilities
In this critical role, you will:
- Engage Members Proactively: Conduct outreach via phone or in-home visits to connect with members, discuss their care plans, and address any questions or concerns. Provide ongoing education to empower members throughout their care journey.
- Coordinate Care Services: Develop and implement care plans in collaboration with healthcare providers, community partners, and members/caregivers. Adapt to changes in member needs or progress to ensure optimal outcomes.
- Facilitate Communication: Act as a liaison between members, care managers, practitioners, and other stakeholders to resolve inquiries, streamline services, and maintain continuity of care.
- Perform Assessments and Screenings: Conduct service assessments and screenings to evaluate member needs, documenting findings in compliance with state and regulatory requirements.
- Maintain Accurate Records: Ensure member records are meticulously documented, updated, and distributed to providers as needed, adhering to contractual and regulatory standards.
- Provide Resource Guidance: Educate members on available benefits and resources, including referrals to care managers, community-based organizations, and disease management programs.
- Uphold Compliance: Follow all policies, standards, and safety protocols, including performing background checks and drug screenings as required for this safety-sensitive position.
- Support Additional Initiatives: Assist with special projects or duties as assigned to enhance care coordination and member support.
Qualifications and Requirements
To excel in this role, you must meet the following criteria:
- Education: High School diploma or GED required. For Arkansas Total Care plan, a Bachelor’s degree in a social science or health-related field is preferred, or a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.
- Experience: 1–2 years of related experience in care coordination, healthcare services, or a related field.
- Licensure/Certification: This position is designated as safety-sensitive in Arkansas and requires a valid driver’s license, child and adult maltreatment checks (before hire and recurring), and a drug screen (at time of hire and recurring). Candidates must reside in Arkansas or a bordering city.
- Travel Requirements: 30% travel is required for in-person member engagements and coordination activities.
- Preferred Qualifications: Experience working with Behavioral Health and Developmental Disabilities (BH/DD) populations is highly desirable. Candidates residing in Independence, Sharp, Izard, or Stone County, Arkansas, are preferred.
