Key Responsibilities

In this critical role, you will:

• Facilitate Member Engagement: Conduct proactive outreach via phone or home visits to engage members, discuss care plans, address concerns, and provide ongoing education about available resources and benefits.
• Coordinate Care Services: Collaborate with healthcare providers, community organizations, and caregivers to implement and adjust care plans based on member needs, ensuring continuity of care and member satisfaction.
• Support Care Management Activities: Assist care management teams in delivering services by documenting member records, performing service assessments, and facilitating referrals to appropriate resources, including Care Managers, Disease Managers, and community-based organizations.
• Address Member and Provider Inquiries: Serve as a liaison between members, providers, and internal teams to resolve inquiries, clarify care plans, and ensure compliance with contractual and regulatory standards.
• Maintain Compliance and Documentation: Adhere to state and federal regulations while maintaining accurate and up-to-date member records, ensuring all documentation meets contractual and compliance requirements.
• Educate and Empower Members: Provide members with essential information about their benefits, available resources, and care options to promote informed decision-making and improved health outcomes.
• Perform Additional Duties: Undertake other responsibilities as assigned to support the overall goals of the care management team and the organization.

Qualifications and Requirements

To excel in this role, you must meet the following criteria:

• Education: A High School diploma or GED is required. For Arkansas Total Care plan, a Bachelor’s degree in a social science or health-related field is preferred, or a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.
• Experience: 1–2 years of related experience in care coordination, healthcare, or a related field is required.
• Licensure/Certification: This position is designated as safety-sensitive in Arkansas and requires a valid driver’s license, a child and adult maltreatment check (before hire and recurring), and a drug screen (at time of hire and recurring). Candidates must reside in Arkansas or a bordering city.
• Travel: This field-based role requires 30% travel.
• Preferred Qualifications: Experience working with Behavioral Health and/or Developmental Disabilities (BH/DD) populations is highly desirable. Candidates residing in Washington, Benton, or Madison County, Arkansas, are preferred.
