Position Overview
We are seeking a dedicated and skilled Center of Excellence Doctor (GP) to deliver comprehensive medical care to patients of all ages, addressing a diverse range of medical conditions. In this pivotal role, you will play a key part in promoting prevention and screening initiatives, enhancing disease control, and ensuring medication adherence. Your responsibilities will include fostering collaborative care, facilitating referrals, and working closely with multidisciplinary teams to provide holistic patient management. Additionally, you will be instrumental in educating patients on self-management, healthy lifestyle choices, and the importance of regular follow-up visits to prevent disease-related complications.
Key Responsibilities
- Patient Care and Assessment: Conduct thorough medical assessments, including detailed history-taking, comprehensive physical examinations, and diagnostic testing to evaluate patients' health status and diagnose medical conditions accurately.
- Diagnostic and Treatment Planning: Order and interpret laboratory tests, imaging studies, and other diagnostic procedures to inform diagnosis and treatment planning, adhering to clinical findings and evidence-based guidelines.
- Chronic Disease Management: Oversee the healthcare of patients with chronic medical conditions through continuous monitoring, medication management, and lifestyle counseling to optimize health outcomes.
- Prevention and Screening: Champion prevention and screening initiatives by identifying individuals at risk of developing diseases and providing early intervention through ongoing monitoring, education, and lifestyle counseling.
- Collaborative Care: Work collaboratively with interdisciplinary teams to develop and implement comprehensive care plans that address the unique needs of each patient.
- Documentation and Reporting: Maintain accurate and comprehensive documentation in medical records, including patient histories, physical examination findings, diagnostic test results, treatment plans, progress notes, referral assessment forms, and dashboards for population health outcomes.
- Patient Navigation and Education: Serve as the immediate point of contact for patients, guiding them through their care journey and educating them on disease management, self-management techniques, healthy habits, physical activity, lifestyle modifications, and medication adherence. Emphasize the importance of preventing complications and adhering to follow-up visits.
- Monitoring and Follow-Up: Regularly monitor assigned patient groups, assess their response to treatment, and make necessary adjustments to their care plans under the supervision of the Most Responsible Physician (MRP). Provide ongoing support through scheduled follow-up appointments to track progress, address concerns, and ensure continuity of care.
- Performance Metrics and Program Development: Achieve and maintain predefined Service Performance Objectives (SPOs) and healthcare outcome measures within your Center of Excellence (COE) group. Contribute to the development of new COE programs by documenting processes and procedures in program manuals to ensure standardized and high-quality care delivery.
- Business Development Support: Collaborate with the business development team to promote patient volume across all COE programs, enhancing the reach and impact of our services.
- Continuous Professional Development: Stay abreast of the latest research, guidelines, and treatment options in general practice by attending relevant conferences, participating in professional development activities, and engaging in lifelong learning to deliver exceptional patient care.
- Additional Duties: Perform other professional duties as requested by your line manager to support the ongoing success and growth of My Clinic.
Qualifications and Professional Requirements
- Education: Hold a Doctor of Medicine (MD) degree from a recognized institution.
