Position Overview

We are seeking a dedicated and detail-oriented Outpatient Pharmacist to join our dynamic healthcare team. Key Responsibilities

The Outpatient Pharmacist will be responsible for a wide range of duties designed to optimize medication safety, efficiency, and patient satisfaction. Your core responsibilities will include:

• Medication Order Verification: Review, interpret, and verify physician orders for accuracy, including dosage, duration, and potential drug-to-drug interactions. Clarify any ambiguities with the prescribing physician to ensure patient safety.
• Medication Preparation and Dispensing: Oversee the preparation of verified medications, perform double-checking procedures, and ensure proper labeling. Calculate drug quantities based on patient-specific factors such as body weight and age to ensure precise dosing.
• Technology Integration: Enter prescription orders into the Computerized Physician Order Entry (CPOE) system within Fakeeh Care’s Health Information System (HIS) before dispensing. Ensure timely patient notifications to minimize wait times at dispensing windows.
• Patient Counseling and Education: Provide comprehensive counseling to patients and caregivers on medication usage, storage, potential side effects, and drug interactions. Empower patients with the knowledge to manage their health effectively.
• Medication Safety and Compliance: Adhere to all medication safety protocols, including proper handling, storage, and disposal of hazardous materials. Identify and manage nearly expired medications in accordance with organizational policies and regulatory requirements.
• Inventory and Record Management: Maintain strict inventory control, secure storage of controlled substances, and accurate record-keeping. Ensure compliance with charging protocols for dispensed medications.
• Regulatory and Professional Standards: Demonstrate proficiency in formulary management, generic substitution identification, and adherence to prescription quantity limitations. Stay current with continuing education programs to maintain licensure and expertise in pharmacy practices.
• Infection Prevention and Control: Follow stringent infection prevention and control standards when handling medications, preparing intravenous (IV) and total parenteral nutrition (TPN) formulas, and interacting with patients.
• Training and Development: Contribute to the training and orientation of new pharmacists, students, and trainees. Share knowledge and best practices to foster a culture of continuous learning.
• Patient and Family Education: Educate patients and their families about health conditions, treatment plans, and preventive measures to promote overall well-being and medication adherence.
• Continuous Improvement: Actively participate in continuous improvement initiatives, support Fakeeh Care’s Person-Centered care values, and adhere to safety protocols. Report any medication errors or adverse drug reactions (ADRs) through the appropriate channels.
• Confidentiality and Cybersecurity: Maintain strict confidentiality of sensitive patient and organizational information. Comply with cybersecurity policies and participate in awareness initiatives to mitigate cyber threats.
• Ethical and Professional Conduct: Uphold Fakeeh Care’s code of conduct, ethical standards, and organizational policies. Complete mandatory education requirements in a timely manner.

Qualifications and Requirements

To excel in this role, you must meet the following qualifications:

• Education: Bachelor’s Degree in Pharmacy from an accredited institution.
• Experience: 0 to 2 years of experience in a hospital or outpatient pharmacy setting.
• Technical Skills: Proficiency in Microsoft Word and Excel, with the ability to adapt to electronic health record (EHR) systems and pharmacy software.
• Language Proficiency: Strong oral and written communication skills in English.
• Licensure: Valid license to practice pharmacy as per regional health regulatory requirements.
