In this full-time role, you will collaborate closely with healthcare professionals, families, and advocates to ensure residents receive the highest standard of care. Your expertise will be instrumental in facilitating counseling sessions, supporting discharge planning, and connecting residents with essential community resources. With a focus on empathy, strong communication, and a deep understanding of aging and healthcare systems, you will help create a resident-centered culture that prioritizes dignity, respect, and holistic well-being.

Key Responsibilities

Your role will encompass a broad spectrum of responsibilities designed to foster a supportive and engaging environment for residents and their families:

• Resident and Family Advocacy: Act as a trusted advocate for residents and their families, ensuring their voices are heard and their needs are met with sensitivity and professionalism.
• Admissions and Onboarding: Lead the process of welcoming new residents and their families, including conducting pre-admission screenings, tours, and assessments to ensure a smooth transition into our community.
• Psychosocial Support: Develop and implement social work programs and services tailored to address the emotional, psychological, and social needs of residents and their families.
• Multi-Disciplinary Collaboration: Work seamlessly within our interprofessional healthcare team to coordinate care plans, participate in Resident and Family Care Conferences, and contribute to program development that aligns with resident needs.
• Educational Leadership: Organize and deliver Family Orientation programs, staff training sessions, and educational seminars to promote awareness and best practices in social work, care planning, and resident advocacy.
• Quality Management and Reporting: Maintain thorough documentation and generate statistical reports to evaluate departmental performance, identify emerging trends, and inform future policy and program enhancements.
• Ethical Integrity: Uphold the highest standards of ethics, morality, and professional conduct, adhering to Extendicare’s Code of Conduct and fostering a culture of trust and transparency.
• Community Engagement: Facilitate resident groups and social activities that encourage connection, engagement, and a sense of belonging within our community.
