In this full-time, on-site role, you will be at the forefront of delivering comprehensive psychosocial support, ensuring patients and their families receive the care and resources they need throughout their healthcare journey. Key Responsibilities:

• Psychosocial Assessment and Care Coordination: Conduct thorough assessments to identify the emotional, social, and financial challenges faced by patients and families. Collaborate with the medical team to design and implement individualized care plans that address these needs holistically.
• Counseling and Support: Provide empathetic counseling services to patients and families, offering guidance and emotional support during critical moments. Deliver timely crisis intervention to mitigate stress and facilitate resilience.
• Resource Navigation: Connect patients and families with essential community resources, social services, and support programs in Jeddah. Ensure seamless access to services that improve quality of life and promote long-term wellbeing.
• Discharge Planning and Continuity of Care: Play an active role in discharge planning to ensure smooth transitions from hospital to home or other care settings. Monitor progress and adjust support strategies as needed to maintain continuity of care.
• Documentation and Advocacy: Maintain accurate and confidential records of interventions, assessments, and progress. Advocate for patient rights, ensuring their voices are heard and their needs are prioritized within the healthcare system.
• Multidisciplinary Collaboration: Participate in multidisciplinary rounds and team meetings to align psychosocial support with clinical goals. Contribute to hospital initiatives aimed at enhancing patient education, social support, and overall wellbeing.

Qualifications and Skills:
