Key Responsibilities

• Interdisciplinary Leadership: Lead or co-facilitate family meetings and care conferences, collaborating closely with medical, rehabilitation, and administrative teams to align goals and ensure cohesive care planning.
• Advance Care Planning: Facilitate critical conversations around advance care planning, documenting living wills, and establishing durable power of attorney for healthcare decisions to honor individual preferences and autonomy.
• Social Determinants of Health: Conduct thorough assessments to identify and address barriers such as housing instability, financial constraints, transportation challenges, and caregiver burden, ensuring holistic care.
• Discharge Planning and Resource Coordination: Facilitate safe and timely transitions for residents by coordinating referrals to home health services, skilled nursing facilities (SNFs), and community programs, while linking families to financial assistance and support networks.
• Education and Advocacy: Educate residents and families about disease processes, treatment options, and available services, while advocating fiercely for their rights and informed decision-making.
• Compliance and Documentation: Maintain meticulous, HIPAA-compliant documentation in electronic medical records, ensuring confidentiality and regulatory adherence at all times.
• Risk Management and Safety: Monitor and address risk factors such as elder abuse, exploitation, or safety concerns, reporting findings per organizational policy to mitigate harm and promote well-being.
• Quality Improvement and Accreditation: Participate actively in quality improvement initiatives and accreditation processes, collecting and reporting data on social work outcomes to drive continuous enhancement of care standards.
• Staff Development: Support the professional growth of colleagues by providing education on psychosocial aspects of care, cultural competence, and best practices in patient advocacy.
